First, in North America the most common system is the sliding foot rests.

Second, I have no idea which one of the two footrest designs descibed is the one I've heard great reviews on. I also have a difficult time picturing the second type separate (left and right footrests with movable top) you described. Any pictures or sketches would be helpful.
