I recently paddled with a friend who had shoulder surgery a few months ago. He had some frayed tendons from an overuse injury. He has been paddling for years and switched to a feathered paddle 2-3 years ago. He doesn't really index the shaft too much with his wrist, he "splits the difference" between both sides.

I paddled close to him, and without saying anything, I observed his technique. His body was not doing the same thing on both side. On one side, he kept his arm pretty low, but on the other side, he lifted his arm much higher. He uses an asymmetrical stroke to get that feathered paddle perpendicular to the direction of movement. I think that everyone who uses a feathered paddle, whether they use their wrist to index the shaft or not, will also use their shoulder to some degree.

It's impossible to say for sure that his stroke caused his overuse injury, but if was a gambling man, I'd put money on it.
