I don't care how safety-proof you make a machine, if you're DUMB enough, you can get by it. How many people put on safety glasses if they're grinding, sanding or drilling? It's real easy to loose an eye. And fingers are probably next in line if not first. I know several people personally, first-hand that are down to counting to nine-and-a-half.
