If your forms are 1/2" the beam should be 14'11" I cut a 1/2" wide slot the length of the end forms into the ends of my box beam and then just slid the end forms in and screwed through the beam to secure them to make the box beam more solid I drilled small holes every foot and a half or so and then filled the beam with expanding foam insulation, just be sure the beam is straight as it dries
