I'd echo some of the above opinions, in that I'd get an opinion from an orthopedic surgeon specializing in surgery of the spine; suspect they'd do more imaging studies than you've had so far, most likely an MRI, and then give you an opinion as to whether exercise/rehab/and or kayaking is the way to go,and whether surgery offers any options.
