Cruising speeds from 2 to 3.5 kts in a 22" beam kayak?! I'd stop worrying about the head thing and go back to basics!
Three suggestions (all already made):
1. Get her a decent paddle! The length suggestions her are good. 210 or maybe even less.
2. Leave her alone and just paddle more! Longer distances tend to ween off bad habits (bad habits use small muscles that tire easily - good form uses bigger torso muscles that can go all day). More water time will also up her strength. If her head is going down at beginning or end of stroke, it's fatigue (physical OR mental. Maybe she's just tired of being corrected constantly! That would probably cause me to hang my head too!).
3. Brent Reitz forward stroke video!!!! Get it, have HER watch it (without commentary) - then she can teach you!
