I've done it exactly once, in a swimming pool and I was very proud of myself! 
On real water, I use the side entry method and leave the skirt off. I don't paddle alone and I rarely swim unintentionally, so the only time I do this is for practice. I should probably practice the inverted entry and putting the skirt on, but I tend to think the time would be better spent working on improving my rolling and bracing skills, to make swimming an even rarer event.
